1. Have the coach or manager go to CYSA Online Registration. and fill out the online registration
2. Click on CYSA Online Registration.
3. Go to the bottom of the page and click on the “ONLINE VOLUNTEER DISCLOSURE FORM”

• This is the same as the Team Official Registration and Risk Management Disclosure form (form 1628).
• Have the coach complete the form online and PRINT IT, SIGN IT.

If the coach or manager does not wish to fill out online they can fill out form 1628, sign it and

• Enclose in an envelope. These forms are strictly confidential; the Davis Registrar is the only league official to see these forms.

4. CPR and First Aid card – one copy of each (front only); required for head coach only
5. Referee license for Metro coaches
6. Color Photo - cut 1”x 1” - Write coach’s last name and the team name on back. No Polaroid photos; No hats or sunglasses are allowed in pictures.

Give all the above to your team Registrar.
