Registering with US Club

Since it is the beginning of the new season, it is important to register your new players ASAP, before they start practicing, for insurance purposes. Weather your team is new or returning, I need to get your current team info so that I can revise all teams.

1. All teams: Please submit to me by e-mail your team name and age group (fall season age) and coaches and managers first and last names, (address, e-mail, and phone numbers are needed if they are a new coach or manager). Once this is done I can get new managers and coaches passwords to add their players to their new team. Also please submit to me a list of your current roster and let me know who is new and who is returning.
2. All new Coaches and Managers: If this is your first season with Davis Legacy you will need to go on line to www.usclubsoccer.org click on “registration” and again on “Staff background check” ASAP. If you are a returning Manager or coach this needs to be done before this August (yearly) since passes are good from August to August. No passes will be issued by US club without this being done first.
3. Managers: You will need to keep in your team binder at all times, the player registration form with original parent/guardians signature and a copy of their proof of birth. You need to have this binder at every practice and game in case a player is injured. You will also need this binder along with a current roster (printable) and all passes to register for us club sanctioned tournaments.
4. New players: Player registration forms are only needed to be completed if it is a new player or if the old player registration forms are unattainable from the previous player’s coach or manager. The previous coach or manager should have passed on the binder along with current pass cards to the new team. These forms can be attained by going to www.usclubsoccer.org under “general forms” and by clicking on “youth player registration and medical treatment authorization form”.
5. Inputting new players into system: Go online again to www.usclubsoccer.org and input your e-mail address and password that was issued to you. If your password has not yet been issued please e-mail me the request to issue you one after you have done your staff background check and have given me your info to add you to the system.(see above under new coaches and managers) Once you have logged on, it will allow you access to your team. Click on the team and click add player if you are wanting to add players to your team, or click on any existing player to revise address’ or phone numbers, or to change jersey numbers. Scan your POB (proof of birth) as a PDF format to your computer and attach the POB to the player in the system and click update when finished.
6. Ordering Pass cards: Once your players and coaches are completed in the system, you can contact me by e-mail letting me know which players you want to order for, or just state that you want it for the whole team. Each team uses their team credit card (if you have one) or a personal Visa or MasterCard or debit card, and that can be reimbursed to you by the team. This info can be sent to me via e-mail (I will need the name on the card, what type of card, card number and 3 digit security code on the back of the card.) If you feel more comfortable calling me in person to give me this info, please feel free to do so. If there are no problems, I usually receive the pass cards 24-48 hours, but please allow yourself plenty of time if you have new players coming from other clubs, as this may take much longer since we may need to wait for players to be released from their old club. (However, once pass cards are requested they are ok to practice). After ordering your cards, I will send you a receipt via e-mail for your records. Once I receive the cards, I will forward them to the team manager or coach, and then they can be printed from your printer on card stock paper, cut, and then add the pictures to them and laminate them.
7. Player transfers: If you are intending to borrow a player from another Davis team, no player loan forms are needed, I just need the other coach or Manager from the borrowing team to contact me via- email approving the request. I will then go into the system and add them to your roster for you to print out. If you are wishing to borrow a player from another club, on the US club website under forms, you can print a player loan form. The other clubs coach or manager and you (manager or coach) need to fill that loan form out, and fax it to us club prior to the event. You can get that fax number from their website, or off of the form. This transfer form along with their player pass and POB need to accompany you to the event, and will be needed at check in for tournaments.
