Davis Legacy Registration Info – NEW Online Registration
Managers & Registrars – You must be familiar with the following SPECIAL DEADLINES…
• Team Registration must be completed at least three weeks prior to the first tournament date.
• Last day to turn in Comp Reg Packets to Davis Registrar – June 1st
• Last day to turn in Metro Reg Packets to Davis Registrar – July 1st
• State Cup Applications must arrive at the CYSA office by November 1st….late entries are not accepted!
• State Cup and Association Cup Rosters are frozen 14 days prior to the first scheduled game for the tournament. Guest Players are NOT allowed for State or Association Cup.
• Travel Papers for out-of-state tournaments are due to Davis Registrar 35 days prior to tournament. (Send email to clmstebbins@sbcglobal.net to request travel papers)
• All CYSA forms are on the www.cysanorth.org website as well as the www.DYSL.org website under the forms section for CYSA registration.
• Registration for both players and adults (coaches and managers) will be done online at the www.dysl.org website under CYSA online registration

Team Accounting Form – Attach form with all team players.

Coach Registration - Each coach needs the following items to be registered:
- 1. Coach Registration & Disclosure Form (CYSA form #1628) – Have each coach complete the online registration at www.dysl.org under CYSA online registration under Risk Management Disclosure form. This form is confidential; the Davis Registrar is the only league official permitted to see this form. If the coach does not have Internet access, please have them full out the form 1628 form and turn it in with your team packet.
- Coach License – No longer required for registration
- FOR METRO COACHES: Copy of their referee license ***MANDATORY THIS YEAR****
- CPR and First Aid card – one copy (front only); required for head coach only
- Color Photo - cut 1”x 1” - Write coach’s last name & team name on back. Coaches may not be wearing hats or sunglasses!!
Player Registration forms– Make sure forms are completed. If printed from the online registration they will be typed into the form. You must provide 2 copies. Parents can print two copies or they may print one copy and make a photocopy. I only need one original inked signature but I need two copies of the form 1601 per palyer Attach following items to each registration form with a paper clip.
- Copy of birth certificate or passport only if the player has not played in the CYSA North district before. Highlight name and birth date in yellow. NO hospital or baptismal certificates are accepted. If birth certificate is not in English, then also enclose a completed CYSA Foreign Document Translation Form (available on CYSA website, www.cysanorth.org)
- Color photo, cut 1”x 1”. Write last name and team name on back. No Polaroid photos;
