How to fill out online Player Registration Form

Print this instruction sheet then go online.

1. Go to CYSA Online Registration.

2. Click the “continue” button.

3. Select the appropriate division. Be sure you select both for age and division (Comp or Metro) and click the “register” button.

4. Please click on both boxes to continue. The second box states that you have read and agree with the Davis Legacy Soccer League Parent Code of Conduct. Please click on the click below the box to read the Parent Code of Conduct before checking the box.

5. Under Locate The Player:

  • If the player has registered for CYSA through DYSL before click “Re-Register”.
  • If the player is new but has had a sibling who has registered for a CYSA team through DYSL, click “New Sibling” This is a time saver for you as you don’t have to reenter information.
  • If the player has never registered with DYSL or with any other club in CYSA North, click “New Player”.
  • Click “continue” once you have made a selection

6. If the player is new:

  • Type in the Player First Name, Last Name and Birth Date (name must match birth certificate).
  • Then type in the players mother’s birth month and day (ex. 1221 is December 21) Do not put in the year! This number makes up part of the player’s ID.
  • Then type in the Home Phone and click “Continue”.
  • You will then see the player information.
  • Fill out the Medical Information, guardian information, address, phone and EMAIL.
    • If you do not have an email address you must fill in 1234@DYSL.org and then retype it in to confirm. You can not continue without an email address.
    • For the “other guardian” section if the other guardian has the same address, click on “copy contact info from Primary Guardian.
    • Click on "Continue".

7. If the player is re-registering or is a sibling, please confirm that the information that is in the system is correct and complete any missing information and click “Continue".

8. Under the payment information the “Check” should be clicked. This fee is artificial and you pay your team whatever amount was agreed upon during your first team meeting. This is a necessary step in the online registration process but really does really apply to teams. Click “Continue”.

9. Please read the next page and you may print a copy for your reference.

  • MOST IMPORTANT: under “Documents That You Must Provide”.
    • Please disregard (your team will determine what your payment will be).
    • 1601 form (click on the section here to print 2 copies of your 1601 form)
      Parent must sign the 1601 form.
    • provide a 1x1 inch picture.
    • if the player is completely new to CYSA or if you are not sure, please provide a copy of their birth certificate or passport.
  • All of these forms and pictures must be given to your team registrar.

10. There is a section where you can provide your feedback on the online registration process.
